COVID-19 Protocols and Waiver
The health and safety of our clients, staff, and community is of utmost importance to us. We have prepared for reopening in the safest way we can while the COVID-19 coronavirus pandemic continues, following the guidance of the California Board of Barbering and Cosmetology and staying up to date on the measures other reputable salons are taking. To keep everyone in our salon as safe as possible and to let you know what to expect during your appointment, we have created the +following protocols, which we may update from time to time as is reasonably advisable or legally required:
Every person entering Maverick West Salon is required to:
Wear a face mask, have their temperature taken at the door, wash their hands upon entering and sign this liability waiver. You should also self-certify that you have not had any symptoms within 2 weeks prior to your appointment.
Each provider at Maverick West will be required to wear a mask, gloves when necessary, wash their hands and disinfect tools, chairs and surfaces using approved disinfectant liquids between every appointment. We have implemented a strict cleaning regimen that includes all public areas per the requirements by the State Board of Cosmetology and California State guidelines.
Maverick West will not be offering: beverages beyond single serving bottled water, magazines, walk-in appointments or a reception area. Clients are encouraged to bring their own reading material and beverage from home if they desire.
Upon arrival to Maverick West you will be asked to wait outside or in your car. We will not have an indoor waiting area available. You are to come alone to your appointment as we can only accommodate those receiving services. Your provider will call and/or text you when it is appropriate for you to enter the salon.
We have made a full list of all regulations and guidelines available to you on our website.
At this time, there will be no penalty for last-minute cancellations due to illness, but please let us know as soon as possible. Appointments are very limited right now, and we want to ensure other clients on our running waitlist have the opportunity to come in.
If you do not respond to our request to confirm your appointment and health status at least 24 hours prior, you may lose your appointment and be charged 50% of the service cost. We appreciate you understanding that we have a waitlist of clients eager to get in for an appointment.
ASSSUMPTION OF RISK/WAIVER: Completion and submission is required as a condition to booking your appointment.
Maverick West Salon and our service providers are taking reasonable precautions to protect you and our other clients, staff, and community, including following guidelines from the Centers for Disease Control and Prevention, San Francisco Department of Public Health, and California Board of Barbering and Cosmetology (collectively, “Industry Guidelines”) with respect to safety precautions, including social distancing and sanitization practices, to do what we can to reduce the spread of COVID-19 coronavirus. Leading up to your appointment, we will continue to monitor local and state government restrictions and any updates to Industry Guidelines and will let you know if the restrictions or guidelines then in place impact the appointment or services you have scheduled.
You understand and agree that you are scheduling your appointment during the worldwide, national, and state-wide emergency of the COVID-19 coronavirus pandemic, associated government shelter-in-place orders, local public health agency restrictions, and Industry Guidelines (collectively, “Pandemic”), each of which constitutes a force majeure event that may make it illegal, impossible, inadvisable, unsafe, or commercially impractical for us to provide services or keep your scheduled appointment, and that we are not liable or responsible for any failure or delay in fulfilling or performing services for reasons outside our reasonable control, including the Pandemic.
Additionally, because the COVID-19 coronavirus is currently understood to be highly contagious and its spread is based on a combination of distance and time, you understand that, although we will take the reasonable precautions specified above to reduce the chances of exposure, it is not possible to guarantee any of our safety or wellness at this time, and when you book an appointment with us, you knowingly and voluntarily assume all risks associated with choosing to engage us to provide nonessential services during the Pandemic.
Finally, if you choose not to comply with our protocols before, during, or after your appointment, we may refuse to provide service without providing a refund.
Limitation of Liability, Waiver, and Assumption of Risk.
Completion and submission is required as a condition to book your appointment.
Maverick West Salon and our service providers aim to inform our clients of the options and potential outcomes of every service we perform during a thorough, scheduled consultation before the service has started.
Chemical treatments have different effects on different hair types and colors and may have unexpected and undesired effects on the skin and scalp. Additionally, satisfaction with results is highly subjective. For both of those reasons, results are not guaranteed. You agree to hold our salon and your stylist harmless from all actions, suits, claims, expenses and damages in the event of undesired results, reactions to chemical treatments, or other unexpected effects.
IN NO EVENT WILL MAVERICK WEST SALON OR ITS OWNER/OPERATOR, SERVICE PROVIDERS, EMPLOYEES, CONTRACTORS, OR OTHER REPRESENTATIVES AND AGENTS BE LIABLE FOR ANY CONSEQUENTIAL, INDIRECT, PUNITIVE, EXEMPLARY, SPECIAL, OR INCIDENTAL DAMAGES ARISING FROM OR RELATING TO THE SERVICES, HOWEVER CAUSED OR ON ANY THEORY OF LIABILITY. OUR TOTAL CUMULATIVE LIABILITY IN CONNECTION WITH THE SERVICES, WHETHER IN CONTRACT OR TORT OR OTHERWISE, WILL NOT EXCEED THE AGGREGATE AMOUNT PAID BY YOU TO US FOR SERVICES PERFORMED WITHIN SIX (6) MONTHS AFTER YOUR APPOINTMENT DATE.
If you would like to have a strand test or patch test performed at least 48 hours before your appointment to confirm the effect on your hair, skin, or scalp, please book a separate consultation.
You understand that chemical treatments may have unexpected and undesired effects on the metals in your jewelry and on the fabric of your clothing. Please remove your jewelry prior to your chemical treatment and be mindful when selecting the clothing you will wear to your appointment.
Your stylist is a state certified cosmetologist and will use reasonable efforts to create your desired results based on information communicated by you. Please disclose all details regarding previous hair treatments and chemical services and be clear about your request, which may include providing photographic or other visual examples.
You agree that we have adequately explained the process and possible results, you have been given the opportunity to make requests and get questions answered, and you have decided to go forward with your scheduled services. In other words, you knowingly and voluntarily assume all risks associated with your scheduled services, and you understand that any further alterations or corrections of color or cut beyond the allotted appointment time will be provided at your expense.
You understand that all service providers at Maverick West Salon are professionals licensed by the State of California, and that by law they have the right to refuse service to any client at any time, if they feel as though their safety or well-being is compromised.
